- Health and Safety violations - Equinox Gym at 131 Dartmouth Street Boston MA 02116 - the staff, GM, cleaning people all have been reported to about ongoing issues by members. There is little or no improvement. In men’s locker room : insect infested shower area , open electrical wires above showers, unclean supply and return HVAC ducts, open used razors left in sinks and showers, lockers not cleaned, used towels and garbage not cleaned up , overflowing shower soaps with hair and pink scum , not code and ADA compliant, etc. They fail to remedy for any period of time. | Case (SR) Type: [Miscellaneous] Referred To: [PUBLIC HEALTH COMMISSION]
